How to Register

You can submit your registration forms via email to and register without the need to attend the surgery. Once we have all of your paperwork together, you will be contacted by the practice’s reception team to arrange a telephone consultation with a GP. You will be registered on the day of your appointment and the registration will not be completed until you have had your consultation with a GP.

If you are over 16 years old please complete and submit ALL pages of  the GMS1 Form AND the Adult Health Questionnaire. You will also need to submit a photographic ID (passport, British driving licence, national ID card) and  proof of your address.

If you are registering anyone under 16 years old please complete ALL GMS1 form AND the Child Health Questionnaire. You will also need to submit either child’s birth certificate or passport. Please note a child can not be registered without a parent/guardian being registered with the practice.

Acceptable Identification: tenancy agreement, mortgage agreement, bank statement, driving licence, passport, birth certificate etc.
