How to Register
Due to Covid-19 pandemic you can now submit your registration forms via email to firstname.lastname@example.org and get registered without the need to attend the surgery. Once registered you will receive a confirmation email/text message.
If you are over 16 years old please complete the GMS1 Form and the Adult registration form. You will also need to submit a photographic ID (passport, British driving licence, national ID card) and a proof of your address.
If you are registering anyone under 16 years old please complete GMS1 form and Child registration form. You will also need to submit either child’s birth certificate or passport. Please note a child can not get registered without a parent/guardian being registered with the practice.

We register new patients on every weekday morning either by booked appointment or during the ‘walk in’ hours (8.30am – 10.30am). There are also Saturday morning appointments available. Please note there is no ‘walk in’ clinic available on Saturdays.

Download, print and complete the two registration forms below (one per family member). Simply print at home and complete at your convenience. Return the completed forms back to the surgery with 2 forms of identification (ID) (a photographic ID and a proof of your address) when you come to register (please note we only register new patients in the morning between 8.30 and 10.30).
Acceptable Identification: tenancy agreement, mortgage agreement, bank statement, driving licence, passport, birth certificate etc.
Until you have submitted your ID you will not be registered at the practice.
