How to Register

You can submit your registration forms in person or via email to Once your paperwork has ben checked, you will be contacted by the practice’s reception team to arrange an appointment with a GP. We would prefer you to have a basic health check done  on the day of you registration, so a face to face appointment is advisable, but we also offer video consultations.  You will be registered on the day of your appointment and the registration will not be completed until you have had your consultation with a GP.

If you are over 16 years old please complete and submit ALL pages of  the GMS1 Form AND the Adult Health Questionnaire. Proof of ID and address documentation are not required for registration, although will be needed for access to online services. 

If you are registering anyone under 16 years old please complete ALL GMS1 form AND the Child Health Questionnaire. You should submit either child’s birth certificate or passport. Please note a child can not be registered without a parent/guardian being registered with the practice.

Acceptable Identification: tenancy agreement, mortgage agreement, bank statement, driving licence, passport, birth certificate etc.
